Please contact BAC to obtain a registration code. |
| Either email Status@bactpa.com, call 1.800.521.2654 (opt. 4), or send a fax to 1.614.863.0184 |

| We will need the following information in order to provide you with a registration code: |

Rendering Physician or Hospital Facility Name, Tax ID Number, and NPI |

Patient: ID Number, Name, Date of Birth and Date of Service rendered |

| In the Portal list, please choose BAC Provider’s Portal and then enter the following information: |
