Current Rates
Rates for all plan to options.
Rate history is helpful if available.
Current Benefits
SBCs, benefit schedules, or plan documents for current plan options.
Census
Include all members - employees and dependents. Name, DOB, gender, address / zip, plan & tier selection.
Additional Helpful Information:
Renewal Rates
Renewal rates are not always available when it is time to start the quoting process. Some carriers will want to see incumbent renewal before firming
Claim Experience or Health Applications
Medical & Rx experience is required for groups over 100 or currently self-funded. Health applications may be needed for small groups.
